Medical Cannabis – Referral Process
Your family doctor or treating specialist can refer you to the clinic. You may also refer yourself to the clinic. If you do a self-referral, we will require a release of information consent from you to contact your family doctor to access your complete health records.
From a regulatory perspective, Caledon Clinic is accountable to the College of Physicians and Surgeons of Ontario, and the medical consultation model is required for treatment.
After a review of medical records received from your referring doctor you will have a consultation with the doctor to assess your current health condition and treatments. It would be very helpful if you could provide pharmacy summaries that address the three months before your consultation. During your consultation, the doctor will create a personalized treatment plan for your needs.
The time from referral to treatment varies depending on factors relating to you or our clinic caseload status. If all the criteria above have been met, the wait time can be as short as two weeks.
